Please help me with a few words/phrases in the H&P 7.4 report?
1. In the first paragraph (toward the end), there is this sentence (I typed exactly what it sounds like): "Patient also reports a five pound waking in the last month." Is that what she is dictating?
2. Under REVIEW OF SYSTEMS: "Patient denies visual loss, presbyopia, epistaxis, dental problems, change in voice, neck swelling or stiffness." Is "presbyopia" the correct word?
3. Under "Patient's cardiac risk factors": "The last stress and casts are unknown as far as the cardiac workout." What is the word or phrase that sounds like "casts"?
